1. A photo of your pet, from the front, with its eyes open
2. Its name
3. Its age
4. The first and last name of the guardian
5. The questions you wish to ask (Maximum 5 questions)
6. The messages you may like to give to your animal.
Finally, please specify whether you would like a remote animal whisper or a semi-direct whisper your animal. This can be done with or without a written report.
